These Terms and Conditions ("Terms") govern your access to and use of services provided by Azzur Counseling Inc. ("the Company"). By engaging with Azzur Counseling Inc., you agree to comply with and be bound by these Terms. Please read them carefully.

Azzur Counseling Inc. offers counseling services to individuals and families, with a Biblical foundation inspired by Matthew 5:16. The Company provides a variety of mental health services, including but not limited to individual counseling, family counseling, and therapeutic services tailored to clients' needs.

You must be at least 18 years of age to access services independently. Minors may access services with the consent of a parent or guardian. Azzur Counseling Inc. reserves the right to deny service to anyone who does not meet eligibility criteria or fails to comply with these Terms.

All counseling sessions are confidential in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and other relevant privacy laws. Personal information shared during sessions will not be disclosed to third parties without your written consent, except as required by law (e.g., risk of harm to self or others).

• Scheduling: Counseling appointments can be scheduled via our website, by phone, or in person.
• Cancellation Policy: Clients are required to give at least 24 hours’ notice if they wish to cancel or reschedule an appointment. Failure to provide such notice may result in a cancellation fee of [insert fee amount].
• Missed Appointments: If you fail to attend a scheduled session without prior notice, you may be charged the full session fee.

• Fee Structure: Fees for counseling services vary based on the type and duration of the session. Specific rates will be communicated before scheduling any services.
• Payment Methods: We accept payment through [list accepted payment methods].
• Payment Terms: Payments are due at the time of service, unless otherwise agreed. Failure to make timely payments may result in the suspension of services.
• Insurance: Azzur Counseling Inc. does not accept insurance at this time. Clients are responsible for full payment of services.

Clients are expected to:

• Provide accurate and complete information about themselves for effective counseling.
• Follow therapeutic recommendations provided by their counselor.
• Actively participate in the counseling process.
• Attend scheduled appointments promptly.

Azzur Counseling Inc. reserves the right to terminate counseling services if:

• You fail to adhere to these Terms.
• Your behavior is inappropriate or disruptive to the therapeutic environment.
• The counselor determines that the services provided are no longer beneficial or appropriate for your needs.

You may also choose to terminate services at any time by notifying your counselor. Termination does not absolve you from the responsibility of paying any outstanding fees.

Azzur Counseling Inc. strives to provide high-quality counseling services, but cannot guarantee specific outcomes. By using our services, you agree that the Company, its counselors, and staff shall not be held liable for any damages or losses resulting from the use or inability to use our services, except in cases of gross negligence or willful misconduct.

Azzur Counseling Inc. reserves the right to modify or update these Terms at any time. Any changes will be effective immediately upon posting on the Company’s website or communication to clients. Continued use of services after changes have been made constitutes acceptance of the revised Terms.

In the event of a dispute arising from or related to these Terms or the services provided, you agree to first attempt to resolve the dispute informally with Azzur Counseling Inc. If the matter cannot be resolved informally, it will be subject to binding arbitration in accordance with the laws of the state of Texas.

These Terms and any services provided by Azzur Counseling Inc. shall be governed by and construed in accordance with the laws of the state of Texas, without regard to its conflict of law principles.
